What BMI actually is
BMI is your weight in kilograms divided by your height in metres squared. That’s the entire formula. In imperial units it’s weight in pounds divided by height in inches squared, multiplied by 703.
It’s a ratio of mass to height. Not fat. Not health. Mass.

Where it came from
The formula was devised in the 1830s by Adolphe Quetelet, a Belgian mathematician and astronomer. He was not a physician and he was not trying to assess anyone’s health. He was studying the statistical properties of populations, looking for the average build of the “average man.”
It sat largely unused until 1972, when the physiologist Ancel Keys tested several weight-height formulas against body fat measurements and found Quetelet’s was the best of a mediocre set for population work. Keys named it the body mass index — and warned explicitly that it was appropriate for population studies rather than individual diagnosis.
That warning got lost somewhere between 1972 and your doctor’s clipboard.
Where BMI works well
Across populations. At the scale of thousands of people, BMI tracks health outcomes usefully. Higher average BMI in a population correlates with higher rates of type 2 diabetes, cardiovascular disease and several cancers. For epidemiology it earns its place.
As a cheap first screen. It costs nothing, needs no equipment, and flags people worth a closer look. A BMI of 38 is telling you something regardless of build.
For tracking yourself over time. If your height is fixed, changes in your BMI are just changes in your weight expressed on a standard scale. The trend is informative even when the absolute number isn’t.
Where BMI breaks down
It can’t tell muscle from fat. This is the big one. Muscle is denser than fat, so a well-trained person carries more mass in the same frame. Athletes in strength sports routinely produce BMIs in the overweight or obese range while carrying single-digit body fat. The formula has no way to see the difference.
It misses the opposite case too. Someone can sit comfortably inside the healthy BMI range while carrying a high proportion of fat and very little muscle — sometimes described as normal-weight obesity. Their BMI reads fine and their metabolic risk doesn’t.
Where the fat sits matters, and BMI can’t see it. Visceral fat, packed around the organs, drives far more cardiometabolic risk than subcutaneous fat on the hips and thighs. Two people at BMI 28 with different fat distribution have meaningfully different risk profiles.
The thresholds aren’t universal. Risk begins at lower BMI values in some populations. The WHO has noted that Asian populations tend to carry a higher percentage of body fat at the same BMI and show raised cardiovascular risk at lower values, which is why several countries use lower cut-points.
Height distorts it. Because height is squared while people don’t scale in two dimensions, BMI runs slightly high for tall people and slightly low for short people compared with actual body fat.
It doesn’t apply to everyone. BMI categories aren’t valid during pregnancy, and children and teenagers use age-and-sex percentile charts rather than the adult bands.
What to check alongside it
BMI is one number. Two more take a minute and tell you considerably more.
Waist-to-height ratio. Measure your waist at the navel and divide by your height in the same units. Under 0.5 is the target — keep your waist under half your height. This picks up central fat, which is the kind that matters most, and it works across body types better than BMI does.
Waist circumference on its own. The WHO treats above 94 cm (37 in) for men and 80 cm (31.5 in) for women as raised risk, and above 102 cm (40 in) and 88 cm (35 in) as substantially raised.
If your BMI reads high but your waist ratio is comfortably under 0.5, the BMI is probably registering muscle. If both are high, the BMI is telling you something worth acting on.
Reading your own result honestly
A BMI of 26 on a lifter who squats twice their bodyweight means something entirely different from a BMI of 26 on someone who hasn’t trained in fifteen years. The number is identical. The situation isn’t.
Use it as one input. Ask what your waist says, what your blood pressure says, what your resting heart rate says, what you can lift and how far you can walk. A single ratio from a nineteenth-century astronomer doesn’t get to be the last word on your health.
Common questions
Is BMI accurate? As a population statistic, yes. As an individual diagnosis, no — and it was never designed to be one. It’s a screen, not a verdict.
Why does my BMI say overweight when I don’t look it? Almost always muscle mass. Check your waist-to-height ratio; if it’s under 0.5, the BMI is picking up lean tissue rather than fat.
What’s a good BMI? Anywhere in 18.5 to 24.9 is classified as healthy. There is no single best point inside that range, and where you sit depends on your build and how much muscle you carry.
Does BMI work for older adults? Less well. Muscle mass declines with age and body composition shifts, so a given BMI represents more fat at 70 than at 30. Some research suggests a slightly higher BMI is protective in older adults.
Is BMI different for men and women? The formula and the thresholds are identical. Women carry more essential body fat than men at the same BMI, which is one more reason the number alone doesn’t describe body composition.
